Setup process


When I sign up for an appointment, Reliable Mental Health Services will enter the information I have provided into their electronic healthcare record system and send me an email with new patient paperwork and patient portal access. I will then need to complete all the new patient paperwork packet. This packet will need to be completed in its entirety and submitted before my appointment is approved on the date I have requested.

After all new patient paperwork has been received, reviewed, and approved I will be set up with an appointment. This appointment will reflect a date of my choice. I will be charged the initial intake price at the time the appointment is set up. If for any reason I am not able to keep the initial appointment, the intake monies collected will stay credited to my Reliable Mental Health Services account until I am able to use it.

I understand an intake appointment is not a promise of treatment or a right for medication. An intake process will be an evaluation of my mental health to see if I qualify for diagnosis, treatment, medication, or other services. I understand that not all diagnosis can be treated in an online setting and that after evaluation the provider can suggest/refer in person treatment instead of online services. I understand that if I am referred to in person treatment it is my responsibility to find local services needed/suggested and not the responsibility of Reliable Mental Health Services. Services are nonrefundable.

I understand that at any time during the process and relationship with Reliable Mental Health Services I am responsible for my safety and wellbeing. I understand how to access my local emergency response system such as calling 911, going to my nearest emergency room for evaluation, and/or calling the national crisis suicide prevention hotline (1-800-273-8255 open 24hr a day).

While telehealth is a convenient option, I understand it is not able to manage emergency and/or crisis situations. These situations need to be managed with in person care at your local emergency room. I understand that if I am in crisis and happen to be in session with my provider at the time, Reliable Mental Health will reach out to my emergency contact or my local authorities for a safety check and in person assessment of the situation. I understand, consent, and agree with this policy.

How to schedule telepsychiatry appointments?


After I have completed all paperwork, I will be set up with an appointment. This appointment link will be sent to me via email or electronic transmission of my choice. When the appointment date arrives, I will be able to click the link and see my provider for services. I understand the importance of my time and those at Reliable Mental Health Services, therefore I agree to log in for my appointment 5 minutes before the start time. I understand online mental health services is like any other healthcare appointment. My provider will be in an environment that follows HIPAA protocols. I understand I need to be in an environment where I am safe, undistracted, and able to share private information. If I am unable to keep an appointment, I will reschedule the appointment at least 24 hours in advance. Reliable Mental Health Services does its best to send reminders for appointments so patients will not forget their appointments.

Reliable Mental Health Services understands emergencies happen and usually without notice. If you have an emergency arise and are not able to make your appointment, please let your provider know as soon as you know you are unable to make it. Remember, your provider has reviewed your care and set this time apart for you. A missed appointment not only means a delay in care for you but also others who need service. There are fees associated with missed or late canceled appointments. I agree that a $25 fee is charged to my account for every missed or late canceled appointment. This fee will be deducted immediately after missed appointment. An appointment will be considered missed/canceled after 10 minutes past the start time of the initial visit or 5 minutes after the start time of a follow up appointment. Charges will be assessed at that time. I understand Reliable Mental Health Services reserves the right to terminate treatment with me due to excessive missed or late cancellation appointments. After my initial assessment I will be able to schedule follow up appointments with my provider per service option chosen.

What are the different service options available?


I understand Reliable Mental Health Services offers two options for services that can be changed or cancelled at any time.

Monthly subscription service
I understand if I have signed up for monthly subscription services, I am allowed to sign up for appointments monthly with my provider, have unlimited HIPAA compliant chats with my provider about medications and coping skills, have access to online content for coping strategies and stress management, and have quicker response time on inquiries. This approach to care is more engaged with me in my mental health journey. Reliable Mental Health Services will fill/refill prescriptions in accordance with state and federal law. That usually means a 3-month prescription sent to the pharmacy of my choice. I understand that if I do not have follow up appointments every 3 months, I will potentially run out of medications due to provider not prescribing without seeing me. I understand and take full responsibility to make and keep appointments to keep medication management. I can cancel services at any time. When I cancel service, I will have access to services until the end of my service month and then I will not be renewed. I can switch plans at any time.

Pay per visit service option
I understand if I have signed up for pay per visit service option, I am allowed to sign up for appointments at will, but I will be required to have follow up appointments at least every 3 months to keep medication management access. I will have limited access to HIPAA compliant chat with my provider to discuss medications and side effects. If medication changes are needed the provider might suggest a follow up appointment be arranged. I can switch services at any time. Since I am a pay per visit client there is no service to cancel if I want to leave. I understand that if I do not have follow up appointments every 3 months, I will potentially run out of medications due to provider not prescribing without seeing me. I understand and take full responsibility to make and keep appointments to keep medication management. Reliable Mental Health Services will fill/refill prescriptions in accordance with state and federal law. That usually means a 3-month prescription sent to the pharmacy of my choice.

Do you offer Insurances?


Reliable Mental Health Services is currently a CASH BASED business. This means it is not accepting insurance currently. However, Reliable Mental Health Services will provide patients with billing that has all the information needed to be turned in for reimbursement. Reliable Mental Health Services will work with you to make sure you have the paperwork needed and the information needed for reimbursement. Reliable Mental Health Services does not guarantee and is not responsible for any reimbursements from your insurance.

Do you provide medications management?


If you and your provider come up with a plan for medication management, your provider will send your prescription to the pharmacy of your choice. If you have insurance with medication benefit you should be able to use that to pick up your medications. If you do not have insurance, please talk with our providers about options such as GoodRX and others. If you are looking for home delivery of medications, ask your provider about options. Reliable Mental Health Services is not affiliated with any pharmacies online or other and have no preferences or paid interest in where your medications are filled.

How you deal with technological issues?


I understand when dealing with technology there can be service issues or glitches that can affect my treatment. I understand if there is a connection issue my provider might ask to move our session to a phone call instead of the scheduled format. This call will be just as through as the online session. I agree to move to a phone call in the instance of technological issues. If the issues persist even over a phone call you will be asked to reschedule the visit at another time.

Messaging

I have selected which option for service I would like to receive from Reliable Mental Health Services. If I have selected the monthly subscription, I understand I have unlimited chat access to my provider. This means, I will be able to send messages and get replies and possible medication changes. If I have selected the pay per visit option, I understand I have limited chat access to my provider who will answer questions about my medications/side effects and will suggest a follow up appointment if medications need to be changed.

What paperwork requests is needed?


Reliable Mental Health Services allows providers to fill out paperwork for patients at their own discretion, however, Reliable Mental Health Services askes that providers not fill out FMLA, State or Federal disability paperwork, or other such paperwork without an established relationship. One session is not sufficient to be considered a full relationship with a provider.

Providers will fill out work/school notices or accommodation letters for appointment days if the need arises.

What services you not currently offered?


Reliable Mental Health Services is currently not treating conditions such as drug/alcohol abuse treatment, stimulant ADHD treatment, benzodiazepine anxiety treatment, or DEA scheduled sleep aides. If you are seeking treatment elsewhere for these services but are wanting supplement mental health support Reliable Mental Health Services will be glad to treat other conditions co-occurring.
